Detailed investigation of the histories of women with breast cancer shows the relative complexity of the symptom picture. Many women do not have the 'classic' symptom of painless lump. Pain or discomfort is the second most frequently reported symptom. Information about symptoms from case notes differed from that obtained directly from the patients in a detailed interview. Suggestions are made as to why a misleading symptom picture of breast cancer continues to be drawn, and we discuss the effect this may have on delay in treatment.
